About the role
About the role
This is a part-time with the potential for full time Support Coordinator role based in Maitland, NSW, with AffinitySC'. As a Support Coordinator, you will play a crucial part in helping NDIS participants access the services and support they need to live independently and to the fullest. You will coordinate and monitor the delivery of tailored support packages, acting as the central point of contact for clients and service providers.
What you'll be doing
Assess the needs of clients and develop individualised support plans
Coordinate and monitor the delivery of services by various providers to ensure client needs are met
Advocate for clients and liaise with service providers, government agencies and other stakeholders
Provide emotional support and guidance to clients and their families
Regularly review and adapt support plans as client needs change
Maintain detailed client records and reporting
What we're looking for
Relevant tertiary qualification in a community services or allied health discipline
Community services or allied health qualifications are acceptable for this position
Demonstrated experience in a similar support coordination or case management role
Excellent interpersonal and communication skills, with the ability to build rapport with clients
Strong organisational and time management skills, with the ability to prioritise and multitask
Empathy, patience and a genuine commitment to supporting the elderly and disabled
Understanding of the disability support sectors and relevant legislation
